6,023 skilled home care patients benefited from services that enabled them to recover from illness or injury safely and comfortably at home.
1,312 hospice patients were supported through the end of life; of those, 399 received care at the Hospice House.
Subsidized care for 922 patients was provided in our skilled home care, supportive care and hospice programs through the generosity of community donors for individuals whose health coverage did not fully reimburse the cost of their care. The total cost of this care was $952,100.
564 individuals are able to live at home safely and with added confidence thanks to AHCH's Supportive Care and Lifeline services.
1,112 maternal and child home visits were made to 188 families through our State funded Family Health Promotion Grant.
646 patients were extended companionship and support from trained vollunteers.
797 family members/caregivers of Hospice patients received bereavement reassurance phone calls by trained Bereavement Volunteers.
84 Hospice House volunteers are providing nearly 149 hours a week supporting patients and families.
40 grieving children from 15 communities received support following the death of a loved one as part of the Program for Grieving Children & Teens, a joint program with St. Mary's Regional Medical Center and Franklin Community Health Network.
86 adult participants in AHCH's Pathways and Guidance grief groups were comforted with bereavement information and support.
