Androscoggin Home Care & Hospice has forged strong partnerships to ensure that the flow of care is smooth, well-planned and responsive to the needs of patients and families.
Working With Local Hospitals
Central to Androscoggin Home Care & Hospice's partnerships with regional hospitals is our commitment to providing nursing liaison support through our Intake Department. Nurses regularly communicate with facility case managers and discharge planners to coordinate a smooth transition to home health services for patients following a hospital or nursing facility stay.
Androscoggin Home Care & Hospice has also developed several home health protocols that respond to hospital specialty care programs. We work closely with the Central Maine Heart and Vascular Institute to address the individualized home care needs for patients following their cardiac treatment or surgery, St. Mary’s Regional Medical Center and Central Maine Orthopaedics to support the recovery of patients served at their new, first of its kind Center for Joint Replacement, and Franklin Memorial Hospital to extend home visits to women giving birth at the hospital through the Home Care Connection Program.
Androscoggin Home Care & Hospice partnerships are also making it possible for area residents to access community grief support services. Guidance through Grief, a collaboration with Central Maine Medical Center, offers adult support groups in Lewiston throughout the year. The Program for Grieving Children & Teens brings support group resources to young people ages 3-18 at sites in Auburn, Mexico and Farmington several times a year.
Working With Local Physicians
Physician's who initiate and oversee every patient's home health care plan, play an integral role in the delivery of care. Androscoggin Home Care & Hospice collaborates with local physicians to enhance communication and continuity of patient care.
Elizabeth White, MD, CMD serves as Androscoggin Home Care & Hospice's Director of Medical Services. Dr. White is board-certified in Family Medicine, with fellowship training and CAQ (Certification of Added Qualifications) in Geriatric Medicine. She is also a Certified Medical Director (CMD) through the American Medical Directors' Association. Dr. White has 15 years of administrative and clinical experience in long-term care settings and has been with Androscoggin Home Care & Hospice since 2002. In her role as Director of Medical Services, Dr. White is involved in oversight of clinical quality, program development and staff education. One of her primary goals is to facilitate physician-agency relationships.
Working with Residential Care and Independent Living Facilities
Throughout our service area there are a number of facilities that provide senior citizens with health and support services in a residential setting. Androscoggin Home Care & Hospice has developed liason programs to deliver skilled home care, hospice and suppportive care to residents in coordination with facility staff and family caregivers.
