A Compassionate and Skilled Care Team

Your Attending Physician, assisted by AHCH's Medical Director, oversees your care plan and the hospice medical services we provide.

A Primary Nurse, trained in pain and symptom control, coordinates your care and serves as a link to the other hospice team members offering care and support.

Hospice Social Workers provide emotional support, counseling and help connect you and your family to needed community resources.

Your Home Health Aide provides personal care and is available to do basic household chores.

Chaplains offer spiritual support to you and your family.

Hospice Volunteers lend compassionate support, practical help and companionship. When ever possible, special efforts are made to provide volunteer support so that patients are not alone in the event that family and friends are not close by.

Bereavement Staff and Volunteers continue to serve a patient's family and caregivers after the loss of their loved one for 13 months. Each year, AHCH holds two memorial services to honor the memory of those who have been served by our hospice program.

While individuals who have been diagnosed with cancer have traditionally been more likely to choose hospice, hospice care is also available to any terminally ill patient with noncancer diagnoses, such as:

  • Advanced Alzheimer's and other dementia
  • Cardiac disease
  • Pulmonary disease
  • Liver disease
  • Renal disease
  • Stroke
  • Neurological disease
  • Significant decline in health status
  • HIV-related conditions
