Preventing Re-hospitalization

Androscoggin Home Care & Hospice has used telehealth monitoring in conjuction with in-home nursing visits for several years with patients who have Chronic Obstructive Pulmonary Disease (COPD) or Congestive Heart Failure (CHF) with positive outcomes.

There has been a steady decrease in re-hospitalization and visits to the emergency department among patients served. As a result, we have incorporated telehealth monitoring into our "best practice care" plans for patients admitted with both acute as well as chronic conditions.

With a variety of peripheral devices to choose from, we are able to customize the patient's care plan. Supported peripherals include:

  • scale
  • blood pressure monitor
  • pulse oximeter
  • glucose meter

How Telehealth Works

Each day, the automated system greets the user with a gentle, friendly voice that guides them through a simple three minute step-by-step- process to collect their health information.

The system can also be set up to ask the patient a series of yes/no questions to further assist clinicians in evaluating their health and provide reminders.

All of the information is then submitted confidentially by standard telephone line or via cell phone tower to Androscoggin Home Care & Hospice for review. The process is easy, secure, and all carried out in the comfort of the patient's home.
