Androscoggin Home Care & Hospice's Skilled Home Care Program offers a full range of high-quality nursing, therapy, social work, personal care, specialty and volunteer services, all tailored to meet the needs of patients as they recover from a short-term illness or injury.
Patients must meet Home Care requirements for Skilled Services - must be homebound and have a skill need that requires home care.
Specialty home care services include:
Chronic Care - certified staff act as coaches to teach and encourage patients how to self-manage their chronic diseases more effectively therefore avoiding unnecessary trips to the emergency room or hospital.
Diabetes Care & Education - certified educators teach patients and their families proper diabetes management including information about medication regimes, lifestyle changes, and nutrition.
High-Risk Falls Prevention - physical and occupational therapists work with adults at high risk for disabling falls in a structured therapeutic and educational program designed to improve endurance, balance, strength and flexibility.
Infusion (IV) Therapy - specially trained nurses provide intravenous therapy to patients in the comfort of home.
Obstetrical, Maternal & Pediatric Care - nurses, physical therapists and occupational therapists provide care for high-risk pregnant women, infants and children of all ages with medical issues.
Oncology Services - IV certified nurses provide care during and after chemo therapy, management of symptoms, side effects and emotional support during the treatment journey.
Rehabilitation and Orthopedic Services - specialized team of orthopedic nurses and therapists provide care to patients undergoing total joint replacement and patients with non-surgical rehabilitation needs such as post-hospitalization services, including, but not limited to respiratory and cardiac needs.
Telehealth Monitoring Services - supplement regular nursing care visits by monitoring a patient's recovery and health status with tools such as in-home blood testing and monitoring of vital signs.
Wound Care/Ostomy Management - specialized nurses work with patients in managing the care of complex wounds, ostomies, and other skin problems to control infection and promote healing.
